PCS will send you the UCSB License info for Mathematica after receiving this form. Please see License Instructions for how to generate your own Mathpass File.
You DO NOT need to fill out this form if you are using the license server.

If you are unable to use the license server or register for your mathpass file through Wolfram,
then PCS will charge at our hourly rate to assist you.

Fee Schedule

There was no license fee last year (2007-2008), thanks to the generosity of the Physics Department and the College of Letters and Science. Please know that we cannot guarantee this will happen again next year (2008-2009).
