What happens when you leave the UCSB Physics Department

1. Fill out the Computer Separation Form (.pdf).
   • Separation form (.doc).
   • Separation form (.txt).
2. It is your responsibility to make a copy of your computer data prior to leaving.
3. We reserve the right to close your accounts and delete your files as soon as you leave.
4. Upon request, we may provide mail forwarding for up to one month if this doesn't negatively impact our systems and operations. We can provide mail forwarding longer on a case-by-case basis.
5. If you have a network connection for a laptop or desktop, please fill out the Computer Removal Form.
